Privacy and cookies

This statement is about the website praktijklastage.nl: what data the website collects, what for, and how long we keep it. It is separate from the privacy declaration about your medical record that you can draw up with the practice as a patient.

Who is responsible

Praktijk Lastage, Geldersekade 57, 1011 EK Amsterdam, is responsible for the processing of personal data through this website. You can reach the practice through the contact form or by post.

Cookies

This website sets no cookies and stores nothing in your browser. There are no visitor statistics, no trackers and no social media or advertising components. Fonts, scripts and images are served from our own server. As there is nothing to ask your consent for, you will not see a cookie notice either.

The contact form

Through the contact form you send us your email address, the subject and your message. We use these details only to answer your question.

Your message is encrypted in your browser with the practice's key before it is sent; only the practice can read it. Our own server receives the encrypted message and keeps it until the practice's record system has collected it. It is then deleted from the server. A message that for whatever reason is not collected is deleted automatically after 90 days. We keep your message no longer than is needed to deal with your question. If you become a patient, only the information in your message that matters for your treatment is added to your record; the retention periods for medical records apply to that.

Registration form and questionnaires

The registration form and the questionnaires are completed only on a personal invitation from the practice. In the registration form we ask for the details needed to register you as a patient: your name, date of birth, citizen service number (BSN), address and contact details, your general practitioner, your health insurer, a contact person and, if you have one, your referral letter. Healthcare providers are required by law to use your citizen service number.

These details are encrypted, sent and deleted from the server in the same way as a message through the contact form. Once received, they become part of your medical record.

To fill in your street and town automatically, the registration form sends your postcode and house number to PDOK, the address lookup service of the Dutch government. PDOK receives no other data.

Technical data

This website and our own server are hosted on servers within the European Union. As with every website, the web server of our hosting provider records which pages are requested, with the IP address, the time and the type of browser. The practice uses these logs only for security and technical maintenance. Our own server also keeps a technical log that is erased after 30 days and contains no IP addresses and no message content.

Your rights

You have the right to see the personal data we hold about you, to have it corrected or deleted, and to object to its processing. To do so, contact the practice. If you are not satisfied with how we handle your data, you can lodge a complaint with the Dutch Data Protection Authority, the Autoriteit Persoonsgegevens.
